When that didn't cure the problem he might have prescribed anti-depressants, the unspoken message being:

 "It's all in your head." 

When the anti-depressants didn't "cure" you it was time for the Orthopedist. This might have been the first time you heard the term "Thoracic Outlet Syndrome". Most likely, he sent you to Physical Therapy. 

When therapy didn't work, it was off to Pain Management.

When the injections didn't work it was back to the Orthopedist.

He explained that at this point he could offer you stronger prescriptions, but you didn't want to live the rest of your life becoming slowly addicted to Percoset and Oxycontin. 

As a last resort he talked about Thoracic Outlet surgery. Cutting away part of your first rib, your collar bone or some of the muscles deep in your neck. 

It was a very painful, complicated procedure and there was a good chance it could actually make things worse. Once you had it, no matter the outcome, there was no going back.

"There's got to be some other alternative", you said. But he couldn't help you any further. "You're just going to have to live with it. I'm sorry. You might try a support group."
